- Must be a Boone County resident
- Gross monthly income for everyone living together as a household must be within 200% of Federal Poverty Guidelines
- Provide name, birthdate, and social security number for everyone living in the household
Services are provided as funds are available. They are subject to annual household limits and evaluations.
