In order to receive services, a client's family income must be under 150% of the Federal Poverty Income guidelines. VAC requires that clients bring these items each time they visit the VAC office:

PROOF OF YOUR CURRENT PHYSICAL BOONE COUNTY ADDRESS (IDS WILL NOT BE ACCEPTED FOR THIS)
Examples: a bill or any piece of mail with your name and current physical address on it (if you are currently living at a homeless shelter, Reality House, or another agency, a letter on the agency’s letterhead from a staff member stating that you are living there will be accepted).

IDENTIFICATION (ORIGINAL…NO COPIES) FOR EVERYONE AT YOUR ADDRESS
Examples: photo id, social security card, birth certificate, Medicaid/Medicare card, printout from DFS with names and social security numbers on it.

PROOF OF ALL INCOME FOR EVERYONE AT YOUR ADDRESS
Examples: paycheck stub, unemployment or child support check stub, printout from DFS (TANF, food stamps), Social Security printout.
